Transportation Assistance Program Application

For $1 per ride, qualified North Liberty residents can call a Yellow Transport taxi to pick up groceries, get to the bank, visit a doctor, connect to a bus or certain other essential quality-of-life trips scheduled Monday through Friday from 7 a.m. to 5 p.m. or Saturday 7 a.m. to 12 p.m.

The focus of the City of North Liberty Transportation Assistance Program (NLTAP) is to assist eligible residents who may – for various reasons – be unable to travel to important destinations in and near North Liberty for essential quality-of-life services. After the application has been received by staff, a home visit appointment will be arranged to introduce the applicant to the program, procure the image for the NLTAP identification card, and discuss other area resources & programs. To apply, fill out the form below OR follow this link to print the form and mail to NLTAP, P.O. Box 77, North Liberty, IA 52317. Printed forms are available upon request at the North Liberty Library and City Hall (WE MOVED360 N Main St).

Residents can be eligible for this program because they don’t have access to vehicle, face financial difficulty, or are permanently or temporarily disabled.

Before applying, please note that NLTAP cannot be used for rides to work, day care drop off/pick up, or college.
