This application is for a Red Fern Dog Park Annual Pass. Passes expire on December 31 of each year and need to be annually renewed. Printed copies of this form are available at North Liberty City Hall (3 Quail Creek Circle) or you can print your own – North Liberty Resident or Non-Resident (these are PDF fillable forms).
After you submit this form, you will receive a confirmation email which includes a copy of your completed application. When your Red Fern Dog Park Annual Pass is ready, City Staff will contact you via email with your balance due and outline options to remit payment and receive your card. If you do not receive an email from the City within ten (10) business days, then please email [email protected]. On occasion, the webform will not notify staff of a new application.
A Red Fern Dog Park Annual Pass for each dog and a photocopy of your completed form will be mailed to you after the application, necessary documentation, and payment have been received. Allow up to ten (10) business days for processing.
